NEW FORMS for Covid-19 Screening
We have created a new online form through our website that will need to be completed by anyone attending a CHMHA event (practice, tryout, game, etc.) Please make sure you fill out the form and submit prior to arrival of each event.
From a mobile device you can find the Form by Selecting "Team Finder" then select your team division, click on Menu and click "Covid-19 Screening Form".
In addition, anyone entering a rink must sign in at the door for contact tracing purposes.
